DESCRIPTION
When a child is entering a classroom, the teacher should receive the extra sticker printed out by the check-in system. That sticker should be attached to this form. This sticker sheet gives the teacher an accurate roster of how many children they have, their names, and parents phone numbers - very useful in case of an emergency or evacuation!
